To schedule an appointment please contact our office at 978-686-2983 or fill out the online appointment request.  Our secretary will update your information or do a temporary registration at the time of your call, so please have your health insurance information available.  We encoourage you to try our online apointment request and registration forms which will allow us to be better prepared for you on your arrival

To save you time at checkin, please  complete the patient registation form and also the apppropriate history forms (see left side of this page) for the problem you are having

At the time of check in, you will be given a copy of our Notice of Privacy Practices and will be asked to sign an acknowlegement statement. In preparation for your visit, you will need to have available or bring the following information with you:

• Health insurance cards
• Referrals, and/or prior authorizations
• Name, address and telephone number of your primary care phsycian
• Previous medical records relating to your eye or ear problems
• List of medical problems and any prior surgery
• List of Medications
• Current eyeglasses, and /or contact lens presciption

If you have had any X-rays, Cat Scans or MRI's pertaining to the reason for your visit, please pick up the actual films and bring them with you to your visit. We advise eye patients to bring sunglasses and a companion to drive you home, as drops may be instilled to dilate your pupils. If you are unable to keep your scheduled appointment, plelase call us in advance of your appointment.
